Making sure you're drinking enough water and getting enough exercise is a really good place to start so if you carry a bottle of water around with you during the day and make sure you take sips all the way through, especially if you feel that your concentration is lagging, you might see an improvement. Also, if you don't do much exercise already see if you can work in a bit more into your daily routine, maybe walk to school if you don't already or take up a sport.

in terms of in lessons try to give yourself the task of asking yourself at various points during the lesson if you understand what's going on. If you don't understand anything your teacher says, put your hand up and ask. If you give yourself the task of always asking when you don't understand something, that might help you stay on track, giving you something to do if you're feeling a bit bored.

It's often boredom that stops you from concentrating so if you can think of any ways of making lessons more interesting for yourself (without distracting anyone else!) that may help. You could do a little extra research on some of the subjects that you're studying which might make you more interested in class and give you some more questions to ask your teacher.

Being aware of the way you learn is also a good way of improving concentration. Perhaps you are a visual person and therefore making notes with little pictures beside them might help your brain to take in more information.
