I start with a global assessment on the student competence in the set language, to see what are their strengths and weaknesses and then tackle each of these from an academic perspective. I offer both speaking classes and grammar based classes, including phrase analysis and construction.
In Spain I used to prepare my own lessons at home, following different books and preparing my own material. But I think that for the languages the method should be different, because the best way to learn a language is practising it (this is why I'm here!). So the best way to proceed, would be speaking all the time in Spanish with the student and trying to correct the pronunciation and the accent.
