I would try to take a student-led approach whereby they inform me what they believe are their strengths and their weaknesses and from this I can determine where focus should be. My lessons would be targeted at younger students who are struggling with the basics of english, whether that be reading and writing, or higher skills along the lines of thinking creatively, analysis and evaluation.
I like to begin by judging whether the student is at, i.e. what level they are currently working towards and the improvement they would like to make and how likely they are to make such an improvement. I start by seeing their basic understanding of concepts and particular topics, for example Poetry within English, before working towards improving this understanding.
