I approach topics my student finds difficult by using a whiteboard to visually draw out a step by step method. After teaching a method, I will then work through a number of exemplar questions with them until they feel confident to approach similar questions on their own.
First, I would ask the student to do a practice paper to see wheree his strengths and weaknesses in that subject are. Then i would focus on those weaknesses until the student is strong enough in that topic to answer questions on it confidently.

I think that the most useful and effective way to teach someone maths is by breaking it down into simple terms, and building this knowledge up again into more sophisticated understanding.
