I mix it up. However, I teach methods and if students understand quickly, I allow them to continue to practice questions to gain fluency. If not, I will show them a different way, usually visual and real-life examples to help students learn.

I recently graduated from the University of Derby with a BA in Creative Writing (2:1), and am currently studying for a Master's degree in Writing for Performance. I give lessons to ages 14+, all the way to undergraduate level. English Literature lessons are taught simply and methodically, whereas Creative writing lessons are more freeform and activity-oriented.
Depending on what the student would like to learn I will start from wherever, they are currently comfortable and take them to the next step, whilst making sure the fundamentals are solid. Face to face lessons could last up to 3 hours depending on the request, however online support from me will be available.
